Inflammation Hot Spots

When most people hear the word 'inflammation' they think of the swelling and redness that accompanies an injury such as a sprained ankle. However, inflammation can also be systemic and caused not by injury but by lack of exercise and a poor diet. Systemic inflammation increases the risk of cancer, hypertension, heart disease, gastrointestinal problems, and many other health conditions. The process of inflammation starts with injury, toxicity, deficiency or emotional distress. One particular deficiency that causes inflammation is a genetic deficiency called MTHFR. This deficiency is relatively common - perhaps the most common genetic deficiency in North America. It is related to the way we manufacture proteins. A sign that we have MTHFR deficiency is elevated homocysteine.

MTHFR deficiency creates pools of homocysteine that ignite inflammation like bonfires in specific tissues of your body. The strategies for reducing this inflammation depend greatly on where the bonfire starts. And, like many forest fires, inflammation often features multiple fronts that require different strategies for quelling its flames.

Burning brain
The delicate tissues of the brain are common victims of inflammation from an MTHFR deficiency. Elevated homocysteine feeds the fire that melts fragile neurons. Movement disorders like Parkinson's disease and dementias such as Alzheimer's disease are associated with elevated homocysteine. Further, inflammation in the brain associated with MTHFR and elevated homocysteine are also implicated in autism, ADD/ADHD, bipolar depression and schizophrenia.

MTHFR deficiency is addressed by increasing folates in the diet. Folates describe a B vitamin from foliage - leafy green vegetables. Actually, all fruits and vegetables contain some folate. Eating the optimum amount of folate can go a long way to filling the deficiency associated with MTHFR and elevated homocysteine. Remember, the optimum number of servings for fruits and vegetables is:

What causes ulcerative colitis?

Many theories exist about what causes ulcerative colitis. People with ulcerative colitis have abnormalities of the immune system, but doctors do not know whether these abnormalities are a cause or a result of the disease. The body’s immune system is believed to react abnormally to the bacteria in the digestive tract.

Ulcerative colitis is not caused by emotional distress or sensitivity to certain foods or food products, but these factors may trigger symptoms in some people. The stress of living with ulcerative colitis may also contribute to a worsening of symptoms.

How is ulcerative colitis diagnosed?

Many tests are used to diagnose ulcerative colitis. A physical exam and medical history are usually the first step.

Blood tests may be done to check for anemia, which could indicate bleeding in the colon or rectum, or they may uncover a high white blood cell count, which is a sign of inflammation. somewhere in the body.

A stool sample can also reveal white blood cells, whose presence indicates ulcerative colitis or inflammatory disease. In addition, a stool sample allows the doctor to detect bleeding or infection in the colon or rectum caused by bacteria, a virus, or parasites.

A colonoscopy or sigmoidoscopy are the most accurate methods for making a diagnosis of ulcerative colitis and ruling-out other possible conditions, such as Crohn’s disease, diverticular disease, or cancer. For both tests, the doctor inserts an endoscope—a long, flexible, lighted tube connected to a computer and TV monitor—into the anus to see the inside of the colon and rectum. The doctor will be able to see any inflammation, bleeding, or ulcers on the colon wall. During the exam, the doctor may do a biopsy, which involves taking a sample of tissue from the lining of the colon to view with a microscope.

Sometimes x rays such as a barium enema or CT scans are also used to diagnose ulcerative colitis or its complications.

What is the treatment for ulcerative colitis?

Treatment for ulcerative colitis depends on the severity of the disease. Each person experiences ulcerative colitis differently, so treatment is adjusted for each individual.

Drug Therapy

The goal of drug therapy is to induce and maintain remission, and to improve the quality of life for people with ulcerative colitis. Several types of drugs are available.

* Aminosalicylates, drugs that contain 5-aminosalicyclic acid (5-ASA), help control inflammation. Sulfasalazine is a combination of sulfapyridine and 5-ASA. The sulfapyridine component carries the anti-inflammatory 5-ASA to the intestine. However, sulfapyridine may lead to side effects such as nausea, vomiting, heartburn, diarrhea, and headache. Other 5-ASA agents, such as olsalazine, mesalamine, and balsalazide, have a different carrier, fewer side effects, and may be used by people who cannot take sulfasalazine. 5-ASAs are given orally, through an enema, or in a suppository, depending on the location of the inflammation in the colon. Most people with mild or moderate ulcerative colitis are treated with this group of drugs first. This class of drugs is also used in cases of relapse.

* Corticosteroids such as prednisone, methylprednisone, and hydrocortisone also reduce inflammation. They may be used by people who have moderate to severe ulcerative colitis or who do not respond to 5-ASA drugs. Corticosteroids, also known as steroids, can be given orally, intravenously, through an enema, or in a suppository, depending on the location of the inflammation. These drugs can cause side effects such as weight gain, acne, facial hair, hypertension, diabetes, mood swings, bone mass loss, and an increased risk of infection. For this reason, they are not recommended for long-term use, although they are considered very effective when prescribed for short-term use.

* Immunomodulators such as azathioprine and 6-mercapto-purine (6-MP) reduce inflammation by affecting the immune system. These drugs are used for patients who have not responded to 5-ASAs or corticosteroids or who are dependent on corticosteroids. Immunomodulators are administered orally, however, they are slow-acting and it may take up to 6 months before the full benefit. Patients taking these drugs are monitored for complications including pancreatitis, hepatitis, a reduced white blood cell count, and an increased risk of infection. Cyclosporine A may be used with 6-MP or azathioprine to treat active, severe ulcerative colitis in people who do not respond to intravenous corticosteroids.

Other drugs may be given to relax the patient or to relieve pain, diarrhea, or infection.

Some people have remissions—periods when the symptoms go away—that last for months or even years. However, most patients’ symptoms eventually return.

What is ulcerative colitis?

Ulcerative colitis is a disease that causes inflammation and sores, called ulcers, in the lining of the rectum and colon. Ulcerative colitis is an inflammatory bowel disease (IBD), the general name for diseases that cause inflammation in the small intestine and colon. It can be difficult to diagnose because its symptoms are similar to other intestinal disorders and to another type of IBD called Crohn’s disease. Crohn’s disease differs because it causes inflammation deeper within the intestinal wall and can occur in other parts of the digestive system including the small intestine, mouth, esophagus, and stomach.

Ulcerative colitis can occur in people of any age, but it usually starts between the ages of 15 and 30, and less frequently between 50 and 70 years of age. It affects men and women equally and appears to run in families, with reports of up to 20 percent of people with ulcerative colitis having a family member or relative with ulcerative colitis or Crohn’s disease. A higher incidence of ulcerative colitis is seen in Whites and people of Jewish descent.

What are the symptoms of ulcerative colitis?

The most common symptoms of ulcerative colitis are abdominal pain and bloody diarrhea. Patients also may experience

* anemia
* fatigue
* weight loss
* loss of appetite
* rectal bleeding
* loss of body fluids and nutrients
* skin lesions
* joint pain
* growth failure (specifically in children)

About half of the people diagnosed with ulcerative colitis have mild symptoms. Others suffer frequent fevers, bloody diarrhea, nausea, and severe abdominal cramps. Ulcerative colitis may also cause problems such as arthritis, inflammation of the eye, liver disease, and osteoporosis. It is not known why these problems occur outside the colon. Scientists think these complications may be the result of inflammation triggered by the immune system. Some of these problems go away when the colitis is treated.

Inflammation

Inflammation or Obesity Which Comes First?

In a 2004 Time magazine article entitled "The Secret Killer", chronic inflammation was targeted as the main issue behind a number of health problems including allergies, joint pain, high blood pressure, heart attacks, ulcers and even cancer. For many years now medical science has backed up these finding, due to continual research, as well as adding obesity to this growing list.

It has long been known that obesity increases inflammation all throughout the body, which begs the question, could inflammation be a cause, not an effect of the obesity?

A study based at the University of Wisconsin, Madison and University of California, San Diego targeted two areas: One being the hypothalamus, a brain region that regulates the body's energy balance and two a so called Master Switch of inflammation that's usually turned off. In lab tests mice that had the Master Switch of inflammation turned "ON" in the brain, and feed a high fat diet, ultimately gained weight. Then another test was performed on mice where genetic engineering turned the Master Switch "OFF" in the hypothalamus of the brain. These subjects were very much protected from becoming obese, and showed it via the results even after being on a high fat diet.

Our bodies need inflammation to fight off infection, the problem comes when this much needed inflammation can't or won't shut off, becoming chronic. The researchers concluded that finding out how to control the switch in the hypothalamus might open the door to fighting obesity and related diseases.

Further studies regarding obesity and inflammation were found by Dutch researchers writing in the journal PPAR Research. The article outlined the complex relationship between fat storage, insulin resistance and secreted proteins. The affect of obesity on tissue in body parts including the liver, scientists findings conclude there is a growing recognition that obesity represents a state of chronic low-level inflammation.

Anemia of Inflammation Affects on Your Health

 

What causes anemia inflammation and chronic disease. (AI/ACD)?

A number of chronic diseases can cause anemia for different reasons.

Infectious and inflammatory diseases. As part of the immune response that occurs with infection and noninfectious inflammatory diseases, cells of the immune system release proteins called cytokines. These proteins help heal and defend the body against infection. But they can also affect normal body functions. In AI/ACD, immune cytokines interfere with the body’s ability to absorb and use iron. Cytokines may also interfere with the production and normal activity of erythropoietin (EPO), a hormone made by the kidneys that stimulates bone marrow to produce RBCs.

Infectious diseases that cause AI/ACD include tuberculosis, HIV, endocarditis—infection in the heart—and osteomyelitis, a bone infection. Sometimes acute infections—those that develop quickly and may not last long—can also cause AI/ACD.

Inflammatory diseases that can lead to AI/ACD include rheumatoid arthritis, lupus, diabetes, heart failure, degenerative joint disease, and inflammatory bowel disease (IBD). IBD, including Crohn’s disease, can also cause iron deficiency due to poor absorption of iron by the diseased intestine and bleeding from the gastrointestinal tract.

Kidney disease. People with kidney disease can develop anemia for several different reasons. For one, diseased kidneys often fail to make enough EPO. In addition, kidney disease results in abnormal absorption and use of iron, which is typical of AI/ACD. Because anemia worsens as kidney disease advances, nearly everyone with end-stage kidney disease has anemia.

People with kidney failure can also develop iron deficiency due to blood loss during hemodialysis, a procedure that removes blood from an artery, purifies it, and returns it to a vein, thereby doing the job that the kidneys no longer can. Low levels of iron and of folic acid—another nutrient required for normal RBC production—may also contribute to anemia in people with kidney disease.

Cancer. AI/ACD can occur with certain types of cancer, including Hodgkin’s disease, non-Hodgkin’s lymphoma, and breast cancer. Like chronic inflammatory disorders and infections, these types of cancer cause inflammatory cytokines to be released in the body. The anemia of AI/ACD can also be made worse by cancer chemotherapy and radiation treatments that damage the bone marrow—where RBCs are produced—and by the cancer’s invasion of bone marrow.

What is anemia?

Anemia is a condition in which the blood has a lower-than-normal number of red blood cells (RBCs). RBCs contain hemoglobin, an iron-rich protein that gives blood its red color and allows RBCs to transport oxygen from the lungs to the tissues of the body. Because RBC numbers are low in anemia, blood hemoglobin levels are also low.

People with anemia may feel tired because their blood does not supply enough oxygen to the body’s organs and tissues. If anemia becomes severe and prolonged, the lack of oxygen in the blood can lead to shortness of breath or exercise intolerance—a condition in which a person becomes easily fatigued during or after physical activity—and eventually cause the heart and other organs to fail.

What is anemia of Inflammation?

AI/ACD is a type of anemia that commonly occurs with chronic, or long-term, illnesses or infections. Cancer and inflammatory disorders, in which abnormal activation of the immune system occurs, can also cause AI/ACD. Some people develop AI/ACD without having any signs of these health problems.

AI/ACD is easily confused with iron-deficiency anemia because in both forms of anemia, levels of iron circulating in the blood are low. Circulating iron is necessary for RBC production. Low blood iron levels occur in iron-deficiency anemia because levels of iron stored in the body’s tissues are depleted. In AI/ACD, however, iron stores are normal or high. Low blood levels occur in AI/ACD, despite normal iron stores, because inflammatory and chronic diseases interfere with the body’s ability to use stored iron and absorb iron from the diet. Certain treatments for chronic diseases may also impair RBC production and contribute to AI/ACD. AI/ACD is the second most common form of anemia, after iron-deficiency anemia, but it is rarely severe.

While AI/ACD can affect people at any age, older adults are especially susceptible because they have the highest rates of chronic disease. AI/ACD is also common among hospitalized patients, particularly those with chronic illnesses.
